Client Waiver & Informed Consent

Please take a moment to read and initial the following information:

• Mobile massage services are available within a 20-mile radius and are provided at an increased service rate to account for travel time, equipment transport, setup, and teardown.
  • The following conditions are required and non-negotiable:
  • Parking: Client must provide guaranteed, free, and easily accessible parking directly adjacent to the residence. Street parking that is metered, permit-restricted, or not reliably available is not acceptable.
  • Environment: Client must ensure a private, quiet, and distraction-free space is prepared prior to the therapist's arrival. This space must be free from pets, children, and other disturbances.
  • Space Requirements: The massage area must allow for the setup of a standard massage table with a minimum of three feet of unobstructed clearance on all sides to enable safe and effective bodywork.
  • Failure to meet any of the above requirements may result in the session being canceled at the therapist’s discretion, with the client held responsible for full payment.
• I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, and improvement of circulation and energy flow.
• If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure/strokes can be adjusted to my level of comfort. I will not hold my therapist responsible for any pain or discomfort I experience during or after the session.
• I understand that the services offered today are not a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.
• I affirm that I have notified my therapist of all known medical conditions and injuries.
• I agree to inform the therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part should I forget to do so.
• I understand that massage is entirely therapeutic and non-sexual in nature. The massage therapist reserves the right to terminate the session immediately, without refund, if I exhibit any behavior that is sexually suggestive, inappropriate, or makes the therapist feel uncomfortable. This policy is in place to ensure a safe and professional environment for both the client and therapist.
• I understand that all personal and medical information I provide will remain confidential and will only be shared with my explicit consent or as required by law. All records will be handled securely in compliance with applicable privacy laws, including HIPAA, if applicable. My privacy is a priority.
• I agree to arrive clean and prepared for my session. I understand that strong perfumes, colognes, or unclean conditions may affect the session’s quality and the therapist’s ability to provide care. I will communicate any concerns or preferences openly with my therapist.
• I understand that my therapist may maintain records of my sessions for professional purposes, including tracking progress and tailoring future treatments. I acknowledge that these records will remain confidential and will not be shared without my explicit consent, except as required by law.
• I give my consent for the therapist to use any techniques they determine to be most beneficial for my unique needs during each session. These techniques may include, but are not limited to, deep tissue massage, lymphatic drainage, passive and assisted stretches, trigger point release, neuromuscular therapy, cupping, red-light therapy and myofascial release. I understand that I may decline any specific technique at any time by informing my therapist.
• I understand that if I receive cupping therapy, it may result in temporary discoloration or bruise-like marks on the skin. These are a normal response to the technique, are not harmful, and typically fade within a few days.
• I understand that if I choose to receive red light therapy, it is a non-invasive treatment using low-level light to support skin health and recovery. While generally safe, I acknowledge that minor side effects such as temporary redness, warmth, or skin sensitivity may occur.
• I acknowledge and agree to the no-show, late arrival, and cancellation policy of Full Spectrum Healing LTD.
  • Cancellations: A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. This allows others the opportunity to schedule. Late cancellations will incur a charge for the full appointment amount, payable before the next session.
  • No-Shows: If you miss an appointment without notice, you will be charged the full session fee to the payment method on file.
  • Late Arrivals: If you arrive late, your session may be shortened to accommodate subsequent clients. Your therapist will determine if there is enough time to proceed with treatment. Regardless of the session length provided, the full session fee applies. Please plan to arrive on time to respect your therapist and other clients.
